are welcome to call us and schedule an appointment as well:

appointments are of course determined by availability and our
doctor's schedules.
Use " NEW CLIENT" form below to register your pet. Once your pet is
pre-registered, you will not have to fill out any long forms when
you get to the office.
If you have multiple pets, download our multipet form at our file
site: http://homepage.mac.com/statvet/Filesharing1.html. (click on
"Multiple pets" link below.) You can download the form as an
Appleworks document or Windows Excel Document, that you can fill
out electronically and email back to us (so we don't have to type
it!) , or view it as a pdf or jpeg (picture) document ( If viewing
as a jpeg, you must down load each page (page 1 for pets 1-5, page
2 for 6-10, page 3 11-15, page 4 for >15). ) or pdf-- fill it
out by hand and fax it back to us --or drop it off in person
so we can meet you, give you a big friendly "Welcome to our furry
family" smile.
