If your pet has a medical condition
we are currently treating, we appreciate frequent communication
from you to let us know how your pet is doing. Many times a medical
diagnosis is a journey, not just a quick conclusion. How the pet
responds or doesn't respond to a certain treatment, will help us
reach our destination, the diagnosis. Use this form to give us
follow-up information, or call us. Your team associate will
communicate with the doctors and will contact you if need be. If
your pet's condition is worsening, we urge you to schedule a
recheck appointment or have the pet seen on emergency at
